Alternative to metronidazole for trichomoniasis — avoid in T1.
Oral
First-line for
- Trichomoniasis (single 2 g dose)
- Bacterial vaginosis
- Amoebiasis
- Giardiasis
Mechanism
Reduced by anaerobes to cytotoxic free radicals → DNA strand breaks.
“Treat partners, abstain 7 days”
- Test of cure at 3 weeks (NAAT)
- Risk of reinfection if untreated partner
Dosing
- Trichomoniasis
- 2 g PO single dose
- BV
- 2 g PO daily x 2 d
Contraindications
- AbsoluteHypersensitivity to nitroimidazoles
- CautionT1 — avoid; blood dyscrasias
Side effects
- Metallic taste
- Disulfiram reaction with alcohol
- Peripheral neuropathy (prolonged)
Fetal & maternal notes
- • Avoid T1 (theoretical teratogenicity)
- • Avoid breastfeeding for 72 h after dose (long half-life)
