Maintenance therapy in BRCA-mutated ovarian cancer.

First-line for

  • 1st-line maintenance in BRCA-mutated advanced ovarian cancer after platinum response
  • Platinum-sensitive relapsed ovarian cancer

Mechanism

Inhibits PARP1/2 → traps PARP on DNA → synthetic lethality in BRCA-deficient (HR-deficient) tumours.

"Olaparib = only in BRCA/HRD"

  • Requires BRCA1/2 or HRD testing
  • MDS/AML risk with prolonged use
  • Contraception mandatory + 6 months post

Dosing

Standard
300 mg BD PO (2 × 150 mg tablets BD)

Contraindications

  • AbsolutePregnancy — teratogenic in animals
  • AbsoluteBreastfeeding + 1 month post
  • CautionSevere hepatic/renal impairment

Side effects

  • Nausea, fatigue
  • Anaemia, neutropenia, thrombocytopenia
  • Pneumonitis (rare)
  • MDS/AML (long-term)

Fetal & maternal notes

  • Teratogenic — effective contraception during and for 6 months post treatment

  • CYP3A4 strong inhibitors/inducers

Clinical pearls

  • 💡 Full blood count monthly for 12 months, then quarterly
